Enclosed I leave fi[illeg.]ed certificate of disability from my attending Physician who is medical examiner of the county in care of those applying for pensions. My first leave was for twenty days commencing Decemebr 23/62, on account of wounds received at Fredericksburg. As by [illeg.]ion of and issued by L. Thomas, Adjutant General.

I sent over my [illeg.] at the expiration of the first twenty days one copy to L. Thomas, Ajt. General, and one copy to [illeg.] Col. of 126 Regt. In a letter received by three or four days after the Col. acknowledged the receipt of my papers. But I have not heard from the Ajt. general, and fear the papers may have been lost or overlooked. Please send over all the necessary papers for this and the former ones sent by [illeg.] from this place Jany. 11th, 1862.
