I hereby certify that I did this ninth day of January carefully examine Captain J. H. Reed of Company D, 126th Regt. Pennsylvania Vol. and find him still unable to travel in consequence of wounds received in the service of the United States.

Personally came before one of the justices of the peace for
said county of Cambria John Lomans, a practical physician who upon his [deleted: ] say that the above is just and correct. Sworn and secured before
me this 9th day of January, 1863.
