Asst. Surgeon D. D. Swift of the 126 Regt. Penn. Vols. (detailed to act in the 91st Regt. Penn. Vols.) having applied for a certificate in which to ground an application for an extension of his leave of absence given near Falmouth Va. January 14th 1863 for fifteen days. I do hereby certify that I have carefully examined this officer, and find that he is afflicted with Dysenteria Chronica. And that in consequence thereof he is, in my opinion, unfit for duty.

I further declare my belief that he will not be able to resume his duties in a less time than twenty days (20) and that to move at present would greatly endanger his life.
